Malegra is primarily prescribed to male patients with erectile dysfunction (ED). The active ingredient ,sildenafil , belongs to the pharmacological class of phosphodiesterase type 5 (PDE5) inhibitors. Sildenafil is used in Malegra to treat ED. This class of medications works by enhancing the physiological effects of nitric oxide, a molecule that occurs naturally and relaxes the penis's muscles.

During sexual stimulation, nitric oxide is released in the corpus cavernosum, the spongy tissue of the penis. This release causes an enzyme called guanylate cyclase to become active, which raises the levels of cyclic guanosine monophosphate (cGMP).
